What is the role of a Care Coordinator?

Our Care Coordinators play a vital role in supporting care homes by acting as a key link between the Primary Care Network (PCN) and local care homes. They help coordinate specialist teams focused on frailty and complex care needs to improve patient outcomes.

They also lead community initiatives to promote cancer screening, vaccination programs, and NHS Health Checks, raising awareness and encouraging early diagnosis.

Each Care Coordinator manages a small group of patients with multiple long-term conditions, providing personalised support to help them better manage their health and reduce the need for hospital services

Enhanced Health in Care Homes

The team support a total of six Care Homes. Each Care Home is aligned to a GP practice within our network, allowing them to receive full support from the team.

Our Priority
We are dedicated to helping people living in care homes enjoy a good quality of life. This involves supporting both the physical and mental health of residents through a comprehensive, multidisciplinary approach.

Care Coordination
Our Care Coordinators serve as the primary point of contact between care home staff and GP Practices. They manage a dedicated email address for care home staff to reach out with medication questions or requests for GP consultations. Additionally, Care Coordinators proactively check in with care homes by phone at least once a week to identify any residents needing medical attention.
