Ageing Well Service

The Ageing Well Service is an additional resource provided by the PCN.
As a PCN we are taking a more proactive role in helping to support patients in ageing well and managing safely at home.

If you are identified as someone who may benefit from input from our new service, we have Social Prescribing Link Workers, Clinical Pharmacists, Care Co-Ordinator’s and other team members who can help support you in ageing well.
The Ageing Well Team have regular support from GP’s as well as frequent Multi-Disciplinary Meetings to ensure all clinical needs are met.

What to expect:

  1. You will be sent a letter describing the service to you and offering you to take part.
  2. A member of the Care Coordinator team will call you to and ask a few questions before proceeding with your referral.
  3. If you wish to engage in the service, a referral will be sent to a member of the Ageing Well Team who is best fit to support your goals and needs.

What happens next?

  1. A member of the Ageing Well Team will contact you and arrange a time to come and visit you at home.
  2. They will conduct a holistic needs assessment. The aim of a holistic needs assessment is to provide a personalised, all-encompassing care plan that addresses all factors influencing a person’s health and well-being, ensuring that care is tailored to meet your unique needs.
  3. After the initial visit, the Ageing Well Team will assess your goals and needs. If necessary, referrals will be made to other services to address any specific requirements.
